  • Fill in the form completely.
  • Follow the instructions

    • DONT USE THE GROUP LETTER TO NAME THE FILE
Email
  1. When you have completed the document, you can come back to this page to email
  2. Click here to open the email client on your computer
    • It should be called "?_?_self.doc"
    • The first ? is your last name and the second ? is your first name.
      • For example if your name is Frodo Baggins then you would name the file baggins_frodo_self.doc
    • The file size should be at least 25K. (To see the file size, right click on the file name and select "properties.")
    • If the file extensions like ".doc" look strange to you then you may want to make some changes to your computer setup. Click here for more details.
  3. Attach the documment following the instructions on you email program
  4. Send the document and attachment
  5. Click here to start the email
