The California Latino 5 a Day Campaign is a public health initiative led by the California Department of Health Services and administered by the Public Health Institute in cooperation with the National 5 A Day Program of the National Cancer Institute. Its purpose is to empower low-income Latino adults and their families to consume the recommended amount of fruit and vegetables and enjoy physical activity every day.

 

Intern Responsibilities:

  • Conduct 5 A Day health education classes for participants.
  • Follow up with participants regarding nutritional or physical activity goals.
  • Prepare and distribute health education materials to participants.

Overview

The San Ysidro Health Center, Center for Latino Research and Health Promotion, is participating in the Health Disparities Collaborative which is meant to transform the systems of care to improve patient health outcomes.  The HDC uses the Planned Care Model and Model for Improvement in the context of Community Oriented Primary Care.  Activities include the improvement of diabetes case management, coordination of care, creation of a diabetes registry, and referral to clinic and/or community resources.

 

Intern Responsibilities:

  • Performing data entry for diabetes patients using the PECS registry
  • Assist provider with completion of PECS forms
  • Follow-up with patients regarding adherence goals.
  • Refer patients to diabetes classes in the clinic and other community resources.

 

 

Overview

 

 

SALSITA is a new childhood obesity prevention & intervention program that screens SYHC pediatric patients (6-11yrs old) for heart disease and diabetes risk factors, namely obesity, eating & activity behaviors, family history and lipid and glucose values.

Intern Responsibilities:

 

  • Conduct the initial appointment phone calls & follow-up calls.
  • Conduct preliminary clinical assessments of patients (e.g. BMI, blood pressure) and administer survey.
  • Deliver tailored health education to child and parents, including nutrition and physical activity modifications.

 SALSA en la Comunidad

Overview

SALSA en la Comunidad seeks to reduce health disparities in the Latino Community by increasing early detection of cardiovascular risk factors and promoting therapeutic lifestyle changes.

 

Intern Responsibilities:

  • Administer questionnaires and surveys at community screenings
  • Administer surveys and conduct follow-up via phone.
