Checklist
Requirements Prior to Beginning
§ Purchase professional liability insurance. The form is found at: http://publichealth.sdsu.edu/research-field/liability.php. This is not medical insurance. This will provide coverage for a claim filed against you by an injured patient, etc. Check the $1,000,000 per incident policy, and pay $35.70 which includes CA surcharge. SYHC does not provide this insurance. The quickest way is to FAX the form and pay with a credit card (1 week). SDSU Graduate School of Public Health requires this insurance to register for 650 credit. The description you should attach is: http://publichealth.sdsu.edu/research-field/mph.pdf (current graduate students) or a page of the description for your project.
§ Provide proof a TB (PPD) test within the past year. SDSU Student Health Services can provide a free TB test. The clinic does not provide the test free to students. Those with a history of TB should provide the results of an X-ray.
§ Fill out a SYHC Volunteer Application. The form is found here.
§ Make an appointment with SYHC Human Resources. Request an appointment with HR with Patricia Quintana. You must submit the application, TB test, and liability insurance to Ruby Ayala, SYHC Human Resources, 1275 30th Street, San Diego, CA 92154 (619-662-4166). You will receive a Volunteer Badge.
§ Register with Project EXPORT (optional). Students receiving training in research are encouraged to register with Project EXPORT. Benefits include access to an Online Classroom with Scholarship and Graduate Program Information, as well as one-on-one academic mentoring with project investigators and staff. Students who complete 50 hours may receive a stipend. Students from underrepresented groups are encouraged to register. Request a Consent Form and Evaluation Form from Patricia Quintana.

§ Complete the Human Subjects Tutorial and forward your passing score to pquintana@projects.sdsu.edu.
§ Register for Field Practice Credit, PH650 (optional). Students interested in earning field practice credit or other academic credit must contact Dr. Greg Talavera (Health Promotion), or consult with their department (outside GSPH) and register before the add deadline. 180 hours are required for 3 units.
The following immunizations and screening tests are general recommendations for persons who have direct patient contact or who come in contact with human biological fluids or tissues:
§ Hepatitis A virus
§ Hepatitis B virus
§ Influenza, seasonal
§ Measles, mumps, rubella (MMR)
§ Polio
§ Tetanus, booster within past 10 years
§ Varicella
Intern responsibilities:
§ Students will attend an initial group training at San
Ysidro Health Center or
§ It is important all students maintain communication with project management for purposes of scheduling clinic days. Students are asked to make known their availability for clinic on Saturday.
§ Students will maintain a Timesheet at the
clinic. Timesheets are collected at the end of each month.
