• Faculty Sponsor's Assurance - All students must submit a copy of
this form signed by their faculty sponsor with their protocol submission. Forms may be mailed or faxed
to the IRB or may be scanned and uploaded to the protocol.

• IRB Fee - For use by investigators who are not affiliated with SDSU and who request or
require the use of SDSU's Institutional Review Board. (Please contact the IRB office for more information regarding this fee.)
• Certificate of Confidentiality - Due to the sensitive nature of data proposed for collection,
additional efforts to protect subject confidentiality may be needed. If necessary, apply for this
certificate through Ms. Olga Boikess, NIMH, 17-C-02 Parklawn Building,
5600 Fishers Lane, Rockville MD 20857.
