CONTINUOUS PASSIVE MOTION

Physiological Effects
  1. Remodeling of collagen along the lines of force is enhanced
  2. Nutrition and metabolic activity stimulates circulation of synovial fluid
  3. Reduces edema by assisting in venous and lymphatic return
  4. Articular cartilage regrowth is achieved by stimulating tissue remodeling
  5. Accelerated healing of articular cartilage, tendons and ligaments
  6. Afferent nerves are activated in muscle, joint and skin
  7. Pain control possibly aided by gate control mechanism.
Indications
  1. Following surgical repair of stable intra-articular orextra-articular joint fractures
  2. After surgery on the anterior cruciate ligament
  3. Following knee arthroplasty, menisectomy, manipulations and jointdebridement.
  4. Joint contractures
  5. Tendon lacerations
  6. Following osteochondral repair
Contraindications
  1. Cases where the device causes an unwanted translation ofopposing bones which can overstress healing tissues.
  2. In conjunction with anticoagulent therapy which can produce anintracompartment hematoma
Preparation of Athlete
  1. Take the athletes shoes off
  2. Make sure that the athlete is comfortable
  3. Tell the athlete what to expect and how to use the "interrupt" switch
Treatment Set-up and Initiation
  1. Measure the athlete1. Thigh: from the ischial tuberosity to the joint line
    2. Lower leg: from the joint line of knee to approx. .25 in. below heel
    3. Adjust carriages accordingly
  2. Position the athlete1. Place leg in the unit making sure the joint line of the knee matches upwith the hinge on the machine.2. Adjust the foot to make sure the tibia is in the neutral position.
  3. Set the ROM1. Immediate post-op ROM should be set according to doctors orders, usually10 - 90 degrees, adjust as patient improves.2. Post acute ROM should be set according to available ROM
  4. Set the speed1. Immediate post-op speed should be set according to doctors orders,usually 1 - 2 cycles per minute, adjust as patient improves.2. Post acute speed can be set initially at 1 - 2 cycles per minute
  5. Treatment time1. Immediate post-op time is usually 6 - 8 hours per day2. Post acute time is usually 1 hour, 3x per day.
Treatment Termination
  1. Turn unit off
  2. Remove athlete from machine
  3. Clean hinges and unit cover
